Community Integration Services
This is a case management service which helps adults with mental health challenges, or mental health and substance abuse challenges (co-occurring disorders), who are struggling to live independently and coordinate needed services. Service recipients can be living on their own or with family, in group homes or they can be homeless.
Through the process of assessment and treatment plan development, interventions are tailored to meet individual needs. Natural supports are incorporated to the degree the service recipient desires. Supports include:
- Identifying medical, social, housing, educational, vocational, financial, emotional and other needs.
- Providing links to individuals, groups, agencies and organizations to address identified needs.
- Providing information, consultation and problem solving assistance.
- Providing information to involved family members, including family support groups.
- Crisis management and discharge planning for long-term success.
- Identifying medical, social, housing, educational, vocational, financial, emotional, natural and peer supports, and other needs.
